New Patient Paperwork

Appointments & Cancellation
New patients need to arrive 15 minutes prior to scheduled appointment.  All established patients need to arrive 5-10 minutes prior to scheduled appointment.  To avoid being charged for cancelled appointment, please notify us at least 48 hours prior to your scheduled appointment. All appointments require a 48-hour cancellation notice.

Due to product costs and product preparation, Sculptra treatments require a $400 per vial deposit to schedule appointment.  Deposits are non-refundable for Sculptra treatments.  An appointment can be moved, and deposit applied to a different date of service if the patient notifies the practice at a minimum of 48 hours (2 business days) before scheduled treatment.

Special Conditions
Please notify our scheduler before reserving your treatments if you are pregnant, or have high blood pressure, special physical concerns, or diabetes. All patients will be asked to complete a brief questionnaire upon check-in.

Late Policy
If arrival is delayed, we will make every effort to accommodate your full appointment, but this is not always possible. Service time may be shortened to avoid delays for other patients but will be charged at full value. Last minute changes to appointments will be honored only if a treatment appointment is available for a later time in the day (of your appointment) and other patients are not inconvenienced.

Silence Cell Phones

With quietness as our main concern, we ask that you please observe our silence cell phone policy. We request that you silence all cell phones before entering the treatment room. All emergency calls can be directed through our front desk. Please understand this is for the comfort of our patients.

Visitors & Children Policy
We are unable to provide services to guests accompanied by children. For the safety of your children and the comfort of other patients, this policy is strictly enforced.

Gift Cards
Sando Aker Aesthetics Gift Cards are available for individual services, treatment services, corporate gifts, specific dollar amounts, products or gifts. Gift cards must be present at the time of the service to be redeemed. Gift cards are not refundable and not redeemable if lost, stolen or altered.

Returns
If you are not satisfied with the merchandise that you have purchased, you may receive a store credit within 7 days of purchase. However, we cannot issue credit for merchandise that has been depleted by 25% or more.

Prices Subject to Change
We are constantly expanding our services to bring you the latest and greatest. Although we make every effort to keep our website and aesthetic services menu updated, please note that prices and services are subject to change at any time. We do reserve the right to refuse service at any time, to anyone, for any reason.
