Unique Approach
Why Choose MPSG for Your Breast Augmentation
“One of the first questions I ask during a breast augmentation consultation is what the patient hopes to achieve. We discuss their goals for size, shape, proportion, and overall appearance. Many patients bring photos of results they find attractive, which can be very helpful in understanding their vision and expectations. Those images allow us to have a more detailed conversation about what is realistic for their body frame and anatomy. My goal is not simply to choose an implant size, but to create a result that looks balanced, natural and fits their lifestyle.”
- Dr. John Aker

“One of my favorite parts of a breast augmentation consultation is helping patients navigate their options. From implant size to incision placement, every decision plays a role in creating a result that feels right for you. My approach is centered on education, communication, and developing a surgical plan that aligns with your anatomy.”
- Dr. Di Beckman

Questions & Answers with Dr. Aker
How do I choose the right breast implant size?
Choosing the right implant size depends on your body proportions and desired outcome. We guide you through the process to ensure a natural, balanced result.
What is the difference between silicone and saline implants?
Silicone implants are filled with a cohesive silicone gel, while saline implants are filled with sterile salt water. We help you select the option that best fits your goals and anatomy during your consultation.
Is it possible to combine breast augmentation with other procedures?
Absolutely. Depending on absence of significant medical problems, breast augmentation surgery is frequently combined with tummy tuck surgery or a breast lift surgery for a “mommy makeover”. A breast lift and implants can be combined for a procedure called a breast augmentation with lift. This results in greater size and reduced sag. In cases where additional volume is not desired, a breast lift without implants is the most ideal choice.
Do saline implants last longer than gel implants?
Failure rate over time is about the same.
Could I lose nipple sensation after my breast augmentation?
It is very unlikely to permanently lose sensation. It is common to have temporary increased or decreased sensation.
If I gain weight, will my implants look different?
This varies from woman to woman. Some have significant change in breast size with weight fluctuations while others have very little change.
I’ve heard my implants might “settle”. What does that mean?
Shortly after surgery, the implants tend to “ride high” due to spasm of the muscle. Over the weeks following surgery, the muscle and breast tissue relax allowing the implant to descend into a more natural position. This process is called “settling”.
Does it matter if I have a Board Certified Plastic Surgeon do my implants?
A Board Certified Plastic Surgeon has completed 6-7 or more years of surgical training after medical school. This is one of the longest and most rigorous training programs of any medical or surgical specialty. Board Certification ensures patients that the training is of the highest quality.
How much does breast enlargement cost?
The price of breast augmentation varies based on the specifics of the procedure. When determining total cost, we factor in the cost of breast implants, the specific techniques utilized, the total time required to complete the procedure, and many other aspects of the surgery.
What is recovery like after breast augmentation?
Most patients return to light activity within a few days and normal routines within 1 to 2 weeks. We provide a structured recovery plan to ensure a smooth healing process.
How soon will I be able to return to work?
For less physical job requirements, 3-5 days, and for more physical job requirements, 5-7 days.
Will my breast implants interfere with a mammogram?
It has never been shown that having implants delays the diagnosis of breast cancer. However, a mammographer can lose the ability to see a small amount of breast tissue more often when the implant is on top of the muscle. It may require additional views to be taken.
Will my scars be noticeable after my surgery?
If the endoscopic technique is used, there will be no scar on the breast at all. Over time, the armpit scar fades and is typically not very noticeable. With standard breast augmentation, there would be scars visible on the breast but these would fade over time.
Can I breast feed after I have breast augmentation surgery?
Yes, it is safe to breast feed. Having surgery does not prevent or reduce the ability to breast feed. However, if you’ve never nursed before surgery, we are unable to know if you are biologically endowed to breast feed.
What is “rippling”?
It is common to feel a slight “crinkly” sensation along the breast fold where the implant comes closest to the surface. In very slender women there may be visible wrinkling on the skin surface, more commonly observed with saline than with gel implants, and variable in location.
I have saggy boobs. Will breast augmentation fix that?
Limited breast sagging is frequently corrected by breast augmentation. A breast lift may be required for greater degrees of sagging.
What is the most common problem I might have after breast augmentation surgery?
Common short term problems include temporary changes in nipple sensation. A very temporary tight sensation in the armpit sometimes occurs when the endoscopic technique is performed. The most common long-term problem is failure, or rupture, of the implant with an average device lifespan ranging from 10-15 years. This occurrence is not in any way painful or harmful to a woman´s health.

Breast Implant Options
Patients have a number of options when it comes to getting the best breast implants for their aesthetic goals. In terms of type, patients can choose between saline breast implants and silicone gel breast implants. Each filler material provides its own benefits and drawbacks in terms of feel, safety, and several other factors.
Implant size is a common concern. It’s less about choosing large breast implants or small breast implants and more about choosing the best option to fit your frame. This helps in creating a natural look. Low, moderate, or high-profile breast implants are available, and these also affect the overall appearance of the results.
There are several choices in breast augmentation procedures to enhance your breast shape and size.
Hidden Scar Breast Augmentation
Hidden Scar Breast Augmentation, also known as Endoscopic Breast Augmentation, surgery is a brief surgical procedure done under general anesthesia. During this breast augmentation procedure, a small incision is made in a fold in the armpit area. Then, an endoscope, which is a lighted tube that has a video camera attached, is used to create a pocket for the implant underneath the chest muscles. The endoscope allows our surgeons to see better so that the pocket is created more accurately. This pocket can be either directly behind the breast tissue or behind the pectoral muscle which is behind the breast tissue. Dr. Aker all prefer to place the implant behind the pectoral muscle to give you the most natural look possible. The upper breast will then have a more natural slope and graceful appearance. What you are left with is a round breast that looks very natural. Most of the time it is extremely difficult to see the armpit scar after it is healed and faded. Because the implant was inserted through the armpit there is no scar left on the breasts at all. This procedure was brought to Indiana and the Indianapolis and Carmel region by Dr. Aker in 1996. Our practice has performed over thousands of endoscopic breast augmentations. Although practiced frequently on the east and west coasts, we have the most experience with this technique in the Midwest.
Peri-Areolar and Inframammary Breast Augmentation
In Indiana, the most common incision sites for breast augmentation are around the lower edge of the areola (peri-areolar) and in the fold below the breast (infra-mammary fold). These locations work well for most surgeons, allowing direct access to the breast tissue and muscle. These techniques also place the implants directly behind the breast tissue or behind the pectoral muscle which is behind the breast tissue. Again, the surgeons at My Plastic Surgery Group prefer to place the implant behind the pectoral muscle to give the most natural-looking breasts possible.
Inframammary Breast Augmentation
The inframammary fold (IMF) technique is one of the most common and trusted approaches for breast augmentation. In this method, the implant is placed through a small incision made in the natural crease beneath the breast—called the inframammary fold.
This technique allows for excellent control over implant placement and symmetry, often leading to more precise and natural-looking results. It’s suitable for both saline and silicone implants and provides the surgeon direct access to the breast pocket, minimizing trauma to surrounding tissues.
The IMF incision typically heals well and is easily concealed beneath the breast, even in swimwear or lingerie. Many patients appreciate the balance of a discreet scar with the ability to achieve a smooth, elegant contour.
